Provides step-by-step guidance to mentoring program administrators and staff in not only monitoring individual mentor-youth matches, but also assessing the strengths and weaknesses of their programs. The assessment tool takes the form of a survey of the youth participants in a mentoring program. Administrators and staff get information about: (1) the potential uses of the youth survey, the qualities measured by the survey, and ways to administer the survey; (2) how to calculate the score of individual surveys and to use the survey to strengthen individual mentor-youth relationships: (3) how to find averages and ranges in survey results in order to determine overall program effectiveness; and (4) how to use the survey over time to measure the extent to which modifications in program practices are resulting in stronger mentor-youth relationships. Program administrators who achieve positive survey results can let their communities know that their programs are fostering strong relationships, and they also can use the positive findings to recruit both mentors and potential participants. Also contains blank survey forms and sample scoring sheets.
